What if GIR is too low?
Increase dextrose concentration or rate, or add enteral feeds. Recheck blood glucose within 30 minutes.
How do I reduce a high GIR safely?
Wean rate or dextrose by 1–2 mg/kg/min steps every 30–60 minutes while monitoring blood glucose.
Can I calculate GIR for multiple IV lines?
Yes. Sum the glucose contributions from each infusion before dividing by weight.
When is insulin indicated?
Insulin drip may be used if GIR <4 mg/kg/min still results in hyperglycemia. Follow unit policy.
