Epinephrine Pediatric Dosing

Calculate the correct epinephrine dose, volume, and auto-injector selection for pediatric emergencies.

0.01 mg/kg IM (anterolateral thigh) every 5–15 min as needed; max 0.5 mg per dose.

Calculated Dose

Dose

0.20 mg

Volume: 0.20 mL of 1 mg/mL solution.

Weight & notes

Weight: 20.00 kg • Weight-based raw dose 0.20 mg (capped at 0.5 mg).

Auto-injector guidance: EpiPen Jr®/0.15 mg auto-injector recommended for 15–30 kg.

Epinephrine is time-sensitive. Draw doses in advance when feasible, monitor for tachycardia and hypertension, and be prepared to repeat as per guidelines.

How to Use This Calculator

1

Identify the emergency scenario

Choose anaphylaxis (IM) or cardiac arrest (IV/IO) to apply the correct dilution.

2

Enter accurate weight

Use actual or best-estimate weight; epinephrine is dose-sensitive.

3

Draw up or prepare auto-injector

Convert mg to mL and prepare syringes or auto-injectors accordingly.

Formula

Dose (mg) = weight (kg) × 0.01 mg/kg (both scenarios).

Based on AHA PALS guidelines and World Allergy Organization recommendations for pediatric anaphylaxis.

Frequently Asked Questions

Why different concentrations?

Intramuscular uses 1 mg/mL (1:1000). Intravenous/IO uses 0.1 mg/mL (1:10,000) to avoid dosing errors.

When should epinephrine be repeated?

Anaphylaxis: every 5–15 minutes as needed. Cardiac arrest: every 3–5 minutes during CPR.

Can I give epinephrine via nebulizer?

Nebulized racemic epinephrine is used for croup, not anaphylaxis. Dose: 0.5 mL of 2.25% in 3 mL NS.

How do I store epinephrine?

Protect from light, store at room temperature, discard if discolored or expired.

What if IV access is not available?

Use IM route for anaphylaxis; for arrest, use IO access or endotracheal dosing per PALS guidelines.
